D.D.Chronic infarction from chronic hematoma

1-Both appear as a hypo dense area.
2-Remember that the infarction is not a space occupying lesion, so when it is in the chronic stage where focal edema is resolved, if you look to the epsilateral lateral ventricle, you will find it increased in size if compared to the contralateral one.
3-While in case of chronic hematoma, it is a space occupying lesion which has a mass effect by itself in its three stages acute, sub acute or chronic.
4-So chronic hematoma will cause narrowing of the epsilateral ventricle if compared with contralateral one.
5-Another difference is that in case of chronic infarction, it is usually be beside the ventricle while chronic hematoma never do that.


Old infarction causing dilatation of the epsilateral lateral ventricle if compared to the contralateral one(negative mass effect) and at the same time it is connected to the former.

Notice here that inspite the chronicity of this sub dural hematoma, but it still causing mass effect on the epsilateral ventricle and it is situated away from the ventricle.
